Steps For Application
- Fill The Form DS-11
The form is a requirement for those who have never had a US passport, had their passport when they were below 16 years, lost their passports, legally changed their name from the one in their previous passport or had their previous passport issued over 15 years ago.
- Provide Proof Of Citizenship
Always carry your US birth certificate, certificate of citizenship, certificate of naturalization or any other documents that can prove that you are a citizen of the US. Provide them in photocopy form. They should be black and white, legible, single-sided and on white paper.
- Provide Proof Of Identity
You have to prove that you are who you claim to be. Documents that prove your identity include; a valid Us driver’s license, US military card, government employee ID, and foreign passports. If you have or are currently undergoing gender changes, you must provide medical proof that you are undergoing or underwent gender transition. Submit a photocopy of your ID that meets the required standards. You also need a photograph that meets the set requirements.
- Identify Special Circumstances
If you are a child of 16 or below, you lost your passport or have any other special issues, you need to fill in more details for your application.
- Submit Your Application
Submit your application including all the necessary requirements and relevant documents.
